Intermittent Registered Nurse Care Manager and Specialty Care

The Care Manager and Specialty Care RN monitors and evaluates patient response to therapy based on Nursing Practice Standards, including monitoring lab results and other clinical data. She/he collaborates with patients to assess and identify needs, issues, care goals, and resources in achieving desired outcomes related to discharge, post hospital recovery and health maintenance/wellness. The Care Manager and Specialty Care RN utilizes patient information databases to identify at-risk Veterans requiring coordination of care, utilization of and participation in shared medical appointments, and use of decision support tools along with critical thinking skills to triage Veterans with symptom related complaints. The Care Manager and Specialty Care RN establishes patient education goals and a nurse run follow-up clinic to obtain positive patient outcomes. The Care Manager and Specialty Care RN collaborates with other team members and the larger team (specialty care, family/caregiver, internal and community-based services involved in providing care to the patient) in developing the patient-centered and holistic care plan. The Care Manager and Specialty Care RN serves as an advocate for patients and is a team player to colleagues as he/she continues to enhance his/her own and the team's professional growth. The Care Manager and Specialty Care RN utilizes approved protocols and guidelines to authorize the provision of care to include, but not limited to:
assessing, ordering, and treating patients experiencing acute and chronic symptoms within the primary setting. The Care Manager and Specialty Care RN monitors and tracks consults in collaboration with the primary and/or specialty care team and assists in insuring the Veteran receives the right care, at the right time, in the right place. The Care Manager and Specialty Care RN utilizes patient education materials to optimize patient understanding regarding their acute and chronic disease processes. Nurse and/or provider follow-up may include services through face-to-face visits and/or by telephone. The Care Manager and Specialty Care RN also supports other areas within the medical center as needed and accompanies patients and/or residents upon transfer to other facilities if necessary. United States Citizenship:

Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant:
The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Experience:
BSN preferred. 2-3 years of nursing experience. 3-5 years Medical Surgical experience. 2-3 years Specialty Care experience. Excellent customer service skills. Commitment to teamwork. Grade Determinations:
The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree and approximately 2-3 year's of nursing practice/experience. Walking, bending, stooping, and some lifting in the direct care of patients. There may also be extended periods of sitting.
